Booking and Cancellation Policy:

✔ Patient safety is of paramount importance; the clinic reserves the right to refuse patients.

✔ We accept patients from ages 18 to 90. Exceptions may be made on a case by case basis.

✔ All patients must be ambulatory.

✔ All patients must fill out the patient consent form which can be found here.

✔ Quebec patients must have a valid RAMQ card in order to open a file. All clinics and public hospitals use the RAMQ number to identify patients.

✔ All patients- residents of Quebec with or without a valid RAMQ number and non-residents- are required to secure their appointment with a valid credit card.

✔ To process payment, we accept cash, Visa or Mastercard. We do not accept American Express or personal checks.

Cancellation Policy:

Upon confirmation of the appointment and once valid credit card information has been received, the patient has 5 (five) business days to cancel the procedure without penalty. Failure to do so within this time automatically grants and authorizes Endovision Plus to charge the patient’s credit card for the entire amount of the procedure booked. Not with standing, the patient shall have 30 (thirty) calendar days to rebook the procedure at no cost with the exception of additional procedures performed on top of the basic procedure charged.

The patient may transfer this credit to a family member or friend, who would wish to have a procedure done within the 30 (thirty) day time period of the original booking. Endovision Plus reserves the right to change prices without notice.
